About the Opportunity

Note: This is a fixed term contract for six months (full time)

As a Care Partner, you will play a central role in coordinating culturally safe, consumercentered care for older people living at home. Working within the Aged Care Standards and organisational guidelines, you will:

  • Conduct assessments and develop, implement, and review individualised care plans
  • Coordinate supports and services that align with consumer goals, preferences, and budget
  • Build strong, respectful relationships with consumers, families, carers, and service providers
  • Complete home safety and risk assessments
  • Maintain accurate administration, data, case notes, and financial records
  • Support social and activity group programs that foster inclusion and wellbeing
  • Provide supervision, mentoring, and practice guidance to designated Support Workers
  • Represent Anglicare NT across stakeholder networks as needed
  • Participate in occasional outofhours work and the OnCall roster

You will be an organised, empathetic, and culturally responsive professional with strong communication and interpersonal skills. You thrive in dynamic environments and value teamwork, continuous improvement, and quality service delivery.

To succeed in this role, you will bring:

  • Qualifications in Nursing, Allied Health, Social Work, Community Services or similar (preferred)
  • Experience in care planning, assessment, and service coordination for older people
  • Ability to work respectfully with Aboriginal and Torres Strait Islander peoples and culturally diverse communities
  • Knowledge of aged care reforms, legislation, quality standards, and the Aged Care Quality and Safety Commission
  • Strong administration, ICT, financial monitoring, and reporting skills
  • Understanding of aged care and community service systems and local providers
  • Ability to supervise, mentor, and support staff in delivering highquality consumercentered care
  • Strong organisational skills and ability to manage competing priorities
  • Availability for occasional afterhours and OnCall work
