About the Program
OutCARE provides post‑release accommodation and intensive case management support to people exiting custody who are homeless or at risk of homelessness. Funded by the Northern Territory Government Department of Justice, the program aims to support positive rehabilitation and reintegration outcomes, reduce reoffending and strengthen community safety.
About the Opportunity
The OutCARE Case Worker provides gender‑specific post‑release case management and accommodation support to adult men exiting custody. Working with participants with complex needs and significant trauma histories, the role delivers culturally appropriate, trauma‑informed support through comprehensive assessment, planning, coordination and evaluation.
The Case Worker is responsible for coordinating prison discharge planning, re‑entry links and community transition supports, while supporting participants to develop skills in life management, housing stability, vocational goals, financial management, self‑esteem and personal development.
This role works closely with Corrections, community service providers and Anglicare NT teams to support safe, respectful and effective reintegration into the community.

Note: Due to the nature of the client group and the objectives of the program, this position has an inherent requirement for the worker to be male‑identified.This requirement exists to enable safe practice, effective engagement and the development of trust and rapport with male participants exiting custody.
