About the Opportunity

This is a fixed-term, full-time position for approximately four months based in Alice Springs.

As an OutCARE Case Worker, you will provide case management and practical support to people who are homeless or at risk of homelessness following release from prison. You will work closely with clients to assess needs, develop action plans, coordinate services, and support successful reintegration into the community.

You will:

  • Conduct client assessments and identifying support needs.
  • Develop and review individual case management plans.
  • Support clients to build skills in life management, employment, financial management and personal development.
  • Coordinate referrals and linking clients to community supports and services.
  • Maintain accurate records, reports and client documentation.
  • Work collaboratively with government and community stakeholders to achieve positive outcomes.

To be successful in this role, you will bring:

  • Qualifications in Community Services, Social Work, Human Services or a related discipline, or equivalent experience.
  • Prior experience in a community services role.
  • Strong communication, interpersonal and conflict resolution skills.
  • Experience managing complex client needs and competing priorities.
  • A commitment to culturally safe, strengths-based practice and working respectfully with Aboriginal and Torres Strait Islander peoples.
