About the Opportunity

As the Moving On Case Manager, you will provide case management, advocacy, assessment, referral and practical support to young people who have left the care of the Department of Children and Families CEO.

You will play a key role in helping young people navigate housing, education, employment, life skills and community connections while administering brokerage funding and the Transition to Independence Allowance (TILA).

You will:

  • Deliver trauma-informed, culturally safe and youth-focused case management.
  • Develop and implement individual support plans with young people.
  • Conduct risk assessments and support young people experiencing complex challenges.
  • Administer brokerage funds and Transition to Independence Allowance (TILA).
  • Advocate for young people and coordinate support with families, carers and service providers.
  • Maintain accurate client records and reporting requirements.
  • Participate in community engagement, training and stakeholder networking activities.
  • Contribute to continuous improvement and quality service delivery

To be successful in this role, you will bring:

  • Qualifications in Community Services, Social Work, Human Services or a related discipline.
  • Experience working with vulnerable or high-risk young people and families.
  • Strong knowledge of trauma-informed practice, strengths-based approaches and case management.
  • Demonstrated ability to manage complex situations and make sound decisions under pressure.
  • Excellent communication, advocacy and stakeholder engagement skills.
  • commitment to culturally safe, strengths-based practice and working respectfully with Aboriginal and Torres Strait Islander peoples.
