About the Opportunity
Note: This is a short term (12 month) contract part-time position (3 days per week)
As Case Manager, you will support young people transitioning from Out of Home Care (OOHC) into supported independent accommodation, helping them build the skills, confidence, and networks needed to thrive independently.
Working within a trauma-informed, culturally safe, and strengths-based framework, you will provide intensive case management, advocacy, outreach, life-skills development, and accommodation support to young people at risk of homelessness.
To be successful in this role, you will bring:
- Qualifications in Human Services, Social Sciences or a related discipline
- Experience supporting vulnerable or high-risk young people and families in community-based settings.
- Strong knowledge of the OOHC system and youth homelessness service sector.
- Demonstrated experience applying trauma-informed, strengths-based case management practices.
- Excellent engagement skills and the ability to work effectively with young people experiencing complex need.
- A commitment to culturally safe and inclusive practice, particularly when working with Aboriginal and Torres Strait Islander peoples and culturally diverse communities.
