About the Position 

The Assistance with Care and Housing program supports older people who are homeless or at risk of homelessness by linking them to the most appropriate range of housing and care services in order to meet their immediate and ongoing needs. The Aged Care Housing worker will be required to work both independently and collaboratively with a broad range of stakeholders to facilitate meeting the permanent affordable and secure accommodation and support goals of vulnerable clients.

To be successful in this role:

  • Experience in working with older people with multiple and changing needs, including mental illness and homelessness
  • Demonstrated casework and case management skills and practice
  • A willingness to go the "extra" mile to support older people to remain living in the community

Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
